This website provides general information about HMR Transportation, ways to contact the company, and access to HMR's designated online Appointment Requests form.

Online appointment requests

The Appointment Requests page embeds a trip request form provided through Bambi at api.hibambi.com. Information entered into that embedded form is submitted through Bambi's service for HMR to review and use in its trip-request and scheduling process. The embedded form is a third-party service and may be subject to Bambi's own privacy, security, and data-handling practices in addition to HMR's applicable procedures.

Information you choose to provide

If you call HMR or use the designated Appointment Requests form, HMR may receive information needed to review and coordinate your transportation request. If you email HMR, ordinary email should be used only for general business inquiries and should not contain patient, medical, or ride-specific information.

Do not send sensitive trip information through ordinary email

Do not send diagnoses, medical records, appointment details, facility names tied to a patient, pickup or destination addresses tied to a patient, mobility information, insurance identification numbers, Social Security numbers, payment-card information, or other sensitive health or financial information through ordinary email. Use HMR's designated process for trip requests.

Technical information
